A Clinical Study on the Neurotic Cases in Korean Urban Community


Abstract
Facing the fact that many papers have been out reporting that the mentally ill in Korea express their psychological conflicts through physical symptoms, resulting in many obstacles in psychiatric practice in Korea, the Author tried to evaluate the degree and modes of the physical expression in urban neurotic cases and to correlate the mode of expression with the relevant sociocultural background.
Fifty five neurotic cases(male; 24, female; 31) were collected from 127 neurotic out-patients in National Mental Hospital, Seoul, during five and half month, from May 1 to October 15, 1977.
1) Mean age was 33.2?9.96 years with the range from 16 to 61 years of age. Those in 20s were the most frequent age group.
2) The group of "specified Neurosis" accounted for 70.9% of all the subjects. Twenty nine point one percent of all subjects were so called "Unspecified neurosis" manifested by vague somatic symptoms without definable anatomico-physiological basis or even in case of the presence of psychological symptoms, not by purely identifiable but very ambiguous, unfamiliar psychological symptoms.
3) All the symptoms were 230, in number. The symptoms could be deviled into two large categories; somatic complaints in 57.8%, and psychological complaints in 422.2%. Mean number of complaints presented by each patient was 4. 2.
4) Intrafamilial interpersonal relationship was the precipitating factor in 32.7%, and those with unidentifiable factors occupied 27.3%.5) All the symptoms could be divided into each body systems; respiratory and cardio-vascular, alimentary, musculoskeletal, and the others. The complaints about musculoskeletal systems were more frequent in male more than female neurotics. The complaints about respiratory and cardiovascular, and alimentary system were more frequent in female than male neurotics.
